  • Description: The Yolo County Law Library is an official U.S. Passport Acceptance Facility. Accepts applications on behalf of the U.S. Department of State for Passport Books and Passport Cards. These include: First-time passports Replacement of lost, stolen, or damaged passports Renewal of passports issued before the applicant was 16 years of age Renewal of expired passports issued more than 15 years agoAll other renewals should be accomplished via mail or online.

  • Requirements: Documents Required:- Form DS - 11- Evidence of U.S. Citizenship (Original or Certified, physical copy): U.S Birth Certificate, Consular Report of Birth Abroad or Certification of Birth, Certificate of Naturalization, Certificate of Citizenship- Photocopy of U.S. Citizenship Evident- Present ID- Photocopy of ID- Passport PhotoChildren under 16 years of age:- In addition to the above documents required. For applicants aged 0-15 years, generally BOTH parents and the applicant/child must be present and provide evidence of parental relationship, such as a birth certificate. If a parent is unable to appear, a Form DS-5303, completed and notarized within the past six months, or other documentation is required.Children 16-17 years of age:- In addition to the above documents required. Children ages 16-17 must apply for a passport in person. Parental awareness is required, which may be evidenced by payment for fees with a parent's check. One parent may be required to be present and to present valid identification if the applicant/child does not have sufficient identification.
